This page exists so law firms can provide the claimant with a direct, choice-focused explanation of the limited co-representation structure before any SSA appointment is signed.
Use the language below verbatim in the firm’s own engagement letter, intake packet, or claimant communication. Do not adapt the wording unless the firm’s counsel approves revised language.

Disclosure to the Claimant — Verbatim
Adam Whalan, EDPNA is a non-attorney representative who may represent you in your SSDI/SSI matter before SSA. If you appoint Adam, he may serve as your primary point of contact for SSA administrative representation.
Our firm represents you, or may represent you, in a separate related matter. Because that matter may involve records, benefits, wage-loss information, settlement issues, or legal positions relevant to your SSDI/SSI claim, we may also have a limited role in coordinating information with Adam and reviewing overlap issues with your authorization.
You may choose whether to appoint Adam, our firm, both, another representative, or no representative before SSA. Adam’s role, our firm’s role, and any SSA appointment or fee documents will be explained before you sign them.
